The paper presents a state of art review of OHSMS in process industries. Major industrial accidents/incidents are described along with its root cause in the recent past. OHSMS application with a systematic approach can eliminate such incidents and accidents, such approach is also being considered for a fiber industry whose case based review has been discussed in this article. The initial approach is to identify gaps between the standard requirements of OHSMS and existing practices of the industry and implementation steps are suggested based on observations and data collected. Also, root cause analysis model for the particular industry is also suggested.en_US
